The 8D or 8 Disciplines methodology is a problem solving approach used in quality management and continuous improvement processes. It consists of eight steps to find out the root cause and effective solution of a problem, with the ultimate goal being to prevent recurrence of the same problem in the future. 
8D methodology is similar to PDCA cycle approach and is widely used in industries such as automotive, aerospace and health care where quality management and continuous improvement are critical to success. 
The main purpose of 8D problem solving method is to identify, correct and prevent problems affecting customer, products and operational efficiency. 8D problem solving method was first introduced and used by Ford motors company. 

8D methodology

How to Solve Any Problem With 8 steps of 8D Methodology?  

There are 8 steps of 8D methodology that follows the logic of PDCA cycle; 
D1: Team formation 
D2: Problem description 
D3: Interim containment action 
D4: Identify the root cause 
D5: Develop the permanent corrective action
D6: Validate the permanent corrective action 
D7: Identify and Implement the preventive action 
D8: Team recognition 

D1: Team formation - 

The first step of 8D methodology is to assemble a team of people who will work together to identify & implement the effective solution of the problem. The team should include top level employees from different area of the organization who have the necessary knowledge and skills to address the problem. The team may be the CFT (Cross Functional Team). 

D2: Problem description - 

The team needs to clearly define the problem and its impact on the organization, customer and stack holders. They should also establish a timeline for addressing the problem. 

D3: Develop the Interim containment action: 

In this step, the team needs to take immediate action to contain the problem and prevent it from spreading. This may involve temporary measures such as stopping production, reworking products or implementing temporary fixes. 

D4: Identify the root cause: 

The 4th step is to identify the root cause of problem by using the data analysis tools such as the 5-why or fishbone diagram. 

D5: Develop the permanent corrective action: 

The team needs to develop a plan for permanent correcting the problem. This may involve redesigning the process, improving training or changing procedures. 

D6: Implement the permanent corrective action: 

In this step, the team needs to implement the permanent corrective actions and monitor their effectiveness. 

D7: Identify & implement the preventive action: 

The team needs to develop a plan for preventing the problem from recurring in the future. This may involve developing new procedures improving training or implementing new controls. 

D8: Team recognition: 

The final step is to recognize the team for their efforts and celebrate their success in solving the problem.
